Add SpamFeedback table migration with timestamp index (FRE-4517)
Create migration to add SpamFeedback table with indexes on:
- userId (user relationship queries)
- phoneNumberHash (anonymized lookup)
- createdAt (time-based queries, requested in FRE-4517)
